In TCM clinical practice, Bu Zhong Yi Qi Tang and Gui Pi Tang are both widely used for strengthening Qi. Both may be appropriate for manifestations of Spleen Qi deficiency, such as fatigue, poor appetite, and a pale complexion. However, their clinical focuses have some notable differences.
Simply put, Bu Zhong Yi Qi Tang focuses on Qi deficiency with sinking of the middle Qi, while Gui Pi Tang focuses on Heart-Spleen deficiency with Qi and Blood deficiency. Understanding this distinction makes clinical formula selection much clearer.
Bu Zhong Yi Qi Tang: Look for “Sinking”
When Spleen and Stomach Qi become deficient, the body may lose its ability to lift and hold things in their proper position, resulting in sinking of the Middle Qi. In addition to fatigue, shortness of breath, poor appetite, and spontaneous sweating, patients may present with uterine prolapse, gastric prolapse, rectal prolapse, chronic diarrhea, or a pronounced sensation of downward pressure or heaviness.
Therefore, when the clinical picture is essentially “Spleen Qi deficiency + sinking,” Bu Zhong Yi Qi Tang comes to mind for TCM practitioners.
Another classic application of Bu Zhong Yi Qi Tang is fever due to Qi deficiency. Patients may experience a low-grade fever or fever that worsens after exertion, accompanied by fatigue, shortness of breath, spontaneous sweating, and poor appetite. This is not simply an excess-heat pattern; rather, it should be understood from the perspective of deficient Middle Qi and failure of Clear Yang to ascend.
Gui Pi Tang: Look at the “Heart” and “Blood”
Although Gui Pi Tang also strengthens Spleen Qi, it places greater emphasis on the development of Qi and Blood deficiency and failure of the Heart Shen (Spirit) to receive adequate nourishment.
If, in addition to fatigue and poor appetite, a patient presents with palpitations, insomnia, vivid or excessive dreaming, poor memory, dizziness, and a pale or sallow complexion, Heart-Spleen deficiency should be considered. In such cases, Gui Pi Tang may be more specifically indicated.
Another important clinical application of Gui Pi Tang is failure of the Spleen to contain Blood. For example, a woman may experience heavy or prolonged menstrual bleeding and, over time, develop fatigue, dizziness, palpitations, and insomnia. In such cases, it is important to consider the relationship between Spleen Qi deficiency, impaired containment of Blood, and Heart Blood deficiency.
One Simple Rule to Remember
Bu Zhong Yi Qi Tang looks for “sinking”; Gui Pi Tang looks for “Heart and Blood deficiency.”
When Spleen Qi deficiency is primarily manifested by a sensation of downward pressure, organ movement, chronic diarrhea, or inability to lift and hold, the treatment principle is to supplement the Middle Qi and raise Yang. Traditionally, this has been the focus of Bu Zhong Yi Qi Tang.
When the dominant manifestations are palpitations, insomnia, poor memory, Blood deficiency, or chronic bleeding, the treatment principle is to supplement Qi, strengthen the Spleen, nourish Blood, and calm the Shen. Practitioners will often use Gui Pi Tang to support the patient in this case.
Therefore, clinicians should not prescribe Bu Zhong Yi Qi Tang simply because a patient reports fatigue, nor should they automatically use Gui Pi Tang simply because a patient has insomnia. Although both formulas address Qi deficiency, the key is to identify the direction and nature of the underlying pattern. Accurate pattern differentiation, followed by an appropriate treatment principle and formula selection, is essential for more precise clinical practice.
在中医临床中,补中益气汤和归脾汤都是非常常用的补气方。两者都能治疗脾气虚引起的乏力、食欲不振、面色淡等问题,但临床重点并不相同。
简单来说,补中益气汤重在“气虚下陷”,归脾汤重在“心脾两虚、气血不足”。掌握这个区别,临床选方就会清晰很多。
补中益气汤:重点看“下陷”
脾胃气虚以后,如果出现升举无力,就容易形成中气下陷。患者除了疲劳、少气、食欲不振、自汗等脾气虚表现,还可能出现子宫脱垂、胃下垂、脱肛、久泻以及明显的下坠感。
因此,临床看到**“脾气虚+下陷”**,就应该想到补中益气汤。
补中益气汤还有一个经典应用,就是气虚发热。患者可能有低热或劳累后发热,同时伴有乏力、少气、自汗、食欲不振等表现。此时并非单纯实热,而应从中气不足、清阳不升的角度辨治。
归脾汤:重点看“心”和“血”
归脾汤虽然同样补脾气,但更强调脾虚以后出现的气血不足和心神失养。
如果患者除了乏力、食欲不振之外,还出现心悸、失眠、多梦、健忘、头晕、面色淡白或萎黄,就要考虑心脾两虚,归脾汤的针对性更强。
归脾汤还有一个重要临床方向,就是脾不统血。例如女性月经过多、经期延长,长期失血以后又出现乏力、头晕、心悸、失眠等表现,就需要考虑脾气虚、统血无力以及心血不足的关系。
临床记住一句话就够了
补中益气汤看“下陷”,归脾汤看“心血”。
脾气虚以后,如果主要表现为下坠、脱垂、久泻、升举无力,治疗方向偏向补中益气、升阳举陷用补中益气汤;如果主要表现为心悸、失眠、健忘、血虚或慢性出血,治疗方向则偏向益气健脾、养血安神用归脾汤。
所以,临床不要看到“乏力”就用补中益气汤,也不要看到“失眠”就用归脾汤。同样是补气,关键还是看病机的发展方向,有针对性的选择,才能收到最好的临床疗效。
